Sponsorship forms are the forms you will use to submit team sponsors. Use the appropriate forms below based on your sport to collect and submit your teams sponsors.

Football Forms & Downloads

Expect to be asked to attend a mandatory parent meeting prior to the start of each season. At this meeting your coach will cover everything about the coming season and collect the following mandatory paperwork:

The Medical History/Physical Form is used for the physical each child must have before they can participate in POP Warner Youth Athletics. Physicals are $10.00 per child.They are typically held at the beginning of the season. However, if your parent elects to use your personal family physician, PLEASE MAKE ABSOLUTELY SURE THAT THEIR PHYSICIAN USES THE OFFICIAL POP WARNER MEDICAL HISTORY / PHYSICAL FORM. NO OTHER PHYSICAL FORMS WILL BE ACCEPTED INCLUDING YOUR DOCTORS PHYSICAL FORM.

Everyone needs to be very clear that NO PLAYER will be allowed to attend or participate in any POP Warner / MAA activity without a completed POP WARNER physical form on file with your team. In order to do so, you each child must have a completed OFFICIAL POP WARNER MEDICAL HISTORY / PHYSICAL FORM on file with your team before August 1st (the first day of practice).

SPECIAL NOTE: This year (2006 Season) POP WARNER is requiring that all players complete their physicals using the POP WARNER physical form. In the past there have been simply too many different forms submitted without the information required by POP WARNER and Our Insurance. Be absolutely sure your parents are advised and use only the POP WARNER physical form if they elect to have their family physician perform their child's physical. UNDER NO CIRCUMSTANCES WILL ANY OTHER FORMS BE ACCEPTED THIS YEAR!

"CLEARED TO PLAY TACKLE FOOTBALL", and INCLUDES THE PHYSICIANS SIGNATURE.
